Each Flower: 1.3 cm (1/2") long, tubular, opening into 5 lobes, lower 3 larger and forming lip, with 2 white blotches towards base, upper 2 narrower, shorter, erect; distinct darker vein down middle of each lobe; calyx has 5 small, pointed lobes; flower-stems thin, 7 mm (1/4") long, arranged alternately with tiny bract at base
Leaves: Few basal leaves, spatula-shaped, 9 mm-2.5 cm (3/8-1") long; several alternate, narrow, pointed stem-leaves, reducing in size towards top of stem
Stems: Unbranched, slender, smooth
Blooming Period: July - August
Habitat: Sphagnum bogs, lakeshores, and wet meadows at low elevations
